How to add header to form in google forms

Assalamu Walaikum,

In this video, I will show you, how to add a header in google forms. let's get started. take a blank page and give the title name. create the question. go to the top head and click the customize theme.

click choose image. you can add an automatic header just select and insert it. just click on the image and select the graphics. and click insert
