How to add title and description in google forms

Assalamu Walaikum, 

In this video, I will show you, how to add a title and description in google forms. let's get started. take a blank form. add the title. go to the right sidebar and click the double t icon. add the title and description. 

and add the question. I will add another title just click the t icon. set the title name. and description. go to preview
